Name of Product: Cottage Bunk Beds
Importer: The Land of Nod, of Northbrook, Ill. They were manufactured in Brazil
Hazard: When screws are missing from the upper bunk's guardrails, the vertical slats on the guardrails can detach and pose the risk of children falling when getting in or out of the upper bunk.
Incidents/Injuries: The firm received two reports of vertical slats detaching from the upper bunk guardrails. No injuries have been reported.
Description: The Cottage Bunk Beds can be configured as bunk beds or as twin beds. The assembled bunk bed dimensions are 81.5 inches (length) by 43.5 inches (width) by 74.5 inches (height). The beds were sold in both white (model #s 236-519 and 188-174) and pine/honey finishes (model #s 459-410 and 172-596) and included a long and a short upper bunk guardrail, each of which should be secured to the inside of the upper bunk bed's side rail with screws. Stickers bearing the model numbers are located on the inside of the lower bunk headboard.
Sold by: The Land of Nod catalog, Web site www.landofnod.com and at The Land
of Nod stores in Illinois and Washington state from January 2006 through January
2009 for about $1,100.
Remedy: Consumers should check that all screws that attach the guardrails to the upper bunk have been installed. If the screws have not been installed, stop using the beds as a bunk bed immediately and contact The Land of Nod to receive new screws free of charge. All known purchasers have been notified of the recall.

Name of Product: Stork Craft Baby Cribs
Manufacturer: Stork Craft Manufacturing Inc., of British Columbia, Canada
Hazard: The metal support brackets used to support the crib mattress and mattress board can crack and break. When one or more support brackets break, the mattress can collapse and create a dangerous gap between the mattress and crib rails, in which a child can become entrapped and suffocate.
Incidents/Injuries: CPSC is aware of 10 incidents in which one or more mattress support brackets broke. In several incidents, the support bracket broke causing the mattress to collapse and create a gap between the mattress and crib rails. CPSC received a report of a toddler who sustained bruises to his forehead. In another incident a child reportedly became entrapped in the gap between the mattress and the drop side rail with no injury.
Description: This recall involves Stork Craft Baby cribs. All cribs with manufacturing
and distribution dates between May 2000 and November 2008 are included in this
recall. The cribs were sold in various styles and finishes. The manufacture date,
model number, crib name, country of origin and the firm's name, address and contact
information are located on the assembly instruction sheet attached to the mattress
support board. The firm's insignia "storkcraft baby" is inscribed on the drop
side teething rail of some cribs.
Sold at: Major retailers including J.C. Penney, Kmart and Wal-Mart stores nationwide and online at Amazon.com, Babiesrus.com, Costco.com and Walmart.com from May 2000 through January 2009 for between $100 and $400.
Remedy: CPSC urges parents and caregivers to immediately stop using the recalled
cribs and find an alternative, safe sleeping environment for their baby. Consumers
should contact Stork Craft to receive a free replacement kit, with new mattress
support brackets.

Name of Product: Jardine Cribs
Manufacturer: Jardine Enterprises, of Taipei, Taiwan. They were manufactured in China.
Hazard: The wooden crib slats can break, creating a gap, which can pose an entrapment and strangulation hazard to infants and toddlers.
Incidents/Injuries: CPSC has received 19 additional incidents of crib slats breaking. In nine of these incidents, consumers reported that their infant or toddler broke the slat while in the crib. In addition, a 22-month-old child fell through the gap between the crib slats when a slat broke. No injuries have been reported.
Description: This recall involves three models of Jardine wooden cribs with the
date codes identified below. Cribs with other date codes are not affected by this
recall. The date code and model number are printed on the label located on the
inside of the bottom rail of the headboard or footboard.

Manufacturer: Munire Furniture Inc, of Piscataway, N.J.
Hazard: The red paint, which is underneath the black finish paint on some of
the cribs and matching furniture, exceeds federal lead limits. If ingested by
young children lead can cause adverse health effects.
Incidents/Injuries: Munire has received one report of a child ingesting the paint. The child was diagnosed with lead poisoning.
Description: The recalled cribs and furniture are made of wood and have a rubbed black finish. Only the "Newport Rubbed Black" cribs and matching furniture manufactured in Indonesia between April 2006 and November 2008 are included in this recall. The model number, date and country of manufacture are printed on the label attached to the side panel of the crib and the date of manufacture is on the back panel of the furniture. Remedy: Consumers should immediately stop using the products and contact Munire Furniture to receive a replacement coupon that will entitle consumers to exchange the products for another product free of charge.

CPSC is aware of the death of a 5-month-old child on August 31, 2008, in Brooklyn,
N.Y. The child became entrapped between the mattress and the stationary side rail
of the convertible crib and suffocated.
This recall involves the Playkids U.S.A. convertible crib/playpen/bassinet/bed with model number PLK-909. "Playkids U.S.A." can be found on the packaging and on a label sewn into the side of the crib. The model number can be found on the packaging. The convertible cribs have a drop side rail, a stationary side rail, a canopy assembly and a bassinet. The sides of the convertible crib, the mattress support, the bassinet, the canopy and the bedskirt are covered in fabric and mesh. The fabric and the mesh come in a variety of colors and patterns.
The convertible cribs, which were made in China, were sold in juvenile product retailers in New York from March 2007 through September 2008 for about $100.
Consumers should stop using the recalled convertible cribs immediately and contact Playkids USA to receive a full refund.

CPSC, in cooperation with Delta
Enterprise Corp, of New York, N.Y., is announcing the voluntary recall today
of about 600,000 drop side cribs. The crib's drop side can detach when the spring
peg is not engaged, which can cause an entrapment and suffocation risk to infants
and toddlers.
If the spring pegs in the base of each leg of the crib are pushed back into the lower track, the pegs can become nonfunctional. This can cause the drop side to detach from the crib and create a gap that can entrap an infant or toddler.
CPSC staff is aware of a death of an 8-month-old child who suffocated when the drop side of his crib detached. CPSC is also aware of one child entrapment and one incident in which the drop side detached due to a disengaged spring peg.
The recall involves all Delta drop side cribs manufactured prior to 2006 that
use "Crib Trigger Lock with Spring Peg" drop side hardware design. The recalled
model numbers and country of origin include: 4340, 4343, 4520, 4600, 4620, 4624,
4625, 4629, 4660, 4665, 4720, 4750, 4751, 4850, 4855, 4857, 4880, 4920, 4925-2,
4925-2B, 4925-6, 4980, and 8605. The model number, country of origin and manufacture
dates are located on top of the mattress support board. The recalled cribs have
Delta's name and address on the mattress support boards and Delta's logo on the
teether rail.
The cribs, which were made in China, were sold at major retailers nationwide from January 2000 through January 2007 for between $33 and $200. Cribs currently sold at retail are not involved.

Parents and caregivers are encouraged to find a safe, alternative sleep environment for their child until the repair kit, with new safety pegs, is safely installed on the recalled cribs.
CPSC would like to remind parents not to use any crib with missing, broken or loose parts. Make sure to tighten hardware from time to time to keep the crib sturdy. When using a drop side crib parents should check to make sure the drop side or any other moving part operates smoothly. Always check all sides and corners of the crib for disengagement. Any disengagement can create a gap and entrap a child. In addition, do not try to repair any side of the crib, especially with tape, wire or rope. (10-21-08)
CPSC, in cooperation with Delta
Enterprise of New York, N.Y., is announcing the voluntary recall to replace
missing safety pegs involving 985,000 drop side cribs. Failure to use or install
safety pegs can cause an entrapment and suffocation risk to infants and toddlers.
When the safety pegs in the base of each leg of the crib are missing from the lower track, the crib locks can disengage and detach if lowered below the peg hole, creating a hazardous gap. This gap can lead to the entrapment and suffocation of an infant or toddler.
CPSC staff is aware of a death of an 8-month-old child who became entrapped and suffocated when the drop side of the crib detached in a reassembled crib where the safety pegs were not installed. CPSC is also aware of two entrapments and nine disengagement incidents in cribs where the safety pegs were missing.
The recall involves all Delta cribs manufactured in Taiwan or Indonesia, with
the "Crib Trigger Lock with Safety Peg" drop side hardware design. These model
numbers and country of origin can be located on the mattress support board label:
4320, 4340, 4500, 4520, 4530, 4532, 4540, 4542, 4550, 4551, 4580, 4600, 4620,

The recalled cribs have date codes ranging from 1995 though December 2005 and one model (4624) was made in 2007. The model numbers are located on the top of the mattress support board.
The cribs, which were made in Taiwan and Indonesia, were sold at major retailers, including Wal-Mart, Kmart and Target.com from January 1995 through September 2007 for about $100. These cribs have also been found to be sold secondhand. Cribs currently sold at retail are not involved.
CPSC and Delta urge parents and caregivers to immediately stop using cribs that are missing a safety peg on any leg of the crib and contact Delta to receive a free, easy-to-install repair kit. The kit will include safety pegs in a bold color and warning labels to be affixed to the mattress board. Consumers with cribs that have all safety pegs installed may continue to use the cribs; however, CPSC recommends that these consumers contact Delta for the repair kit for future assembly purposes.

Name of Product: "KVIBY" Chests
Manufacturer: IKEA Home Furnishings, of Conshohocken, Pa. They were manufactured in Denmark.
Hazard: The glass drawer knobs on the chest can break either during assembly
or in use, posing a laceration hazard to consumers.
Incidents/Injuries: IKEA has received three reports of laceration injuries requiring medical attention. An additional 11 injuries occurred outside of the United States.
Description: The recalled KVIBY Chest is white with 4 drawers and 6 glass knobs. The recall includes chests with article number 201-080-90 and a date stamp of 0817 (first two digits indicate the year; the second two digits indicate the week) or earlier. The article number and date stamp are located on a label attached to the underside of the chest. KVIBY chests with a date stamp of 0818 and later have improved replacement knobs and screws and are not included in the recall.
Sold at: IKEA stores nationwide from August 2007 through July 2008 for about $300.
Remedy: Consumers should protect their hands and carefully remove the glass knobs. Contact IKEA to receive free replacement knobs and screws by mail.

Distributor: Studio RTA, of Pico Rivera, Calif. They were manufactured in China.
Hazard: The stability of the stands does not meet industry standards to prevent TV tip-over, posing a risk of injury or death to consumers. No injuries or incidents have been reported.
Description: This recall involves four different TV stand models. Model numbers are printed on the packaging and instruction's sheet.
"E Series" Model 060331: The product consists of two glass shelves, a lower shelf
made of metal and black leg and frame components. - "Fierro" Model 402875: This product was sold exclusively by Best Buy. The product consists of three glass shelves with black leg and frame components.
- "Madison DLP" Model 060176: The product consists of three glass shelves with dark wood grain finish frame components, black legs and a black fiberboard back panel.
- "Madison 3000" Model 060090: The product consists of three glass shelves with dark wood grain finish frame components, black legs and a metal mesh back panel.
Sold by: Best Buy (model 402875) and other retailers nationwide from May 2004
through August 2008 for about between $220 and $260.

Name of Product: SmartSpace™ Papasan Chairs
Importer: Jo-Ann Stores Inc., of Hudson, Ohio. They were manufactured in China.
Hazard: The chairs were sold without a warning label instructing consumers to
lock the chair's legs before sitting. This poses a fall hazard to consumers.
Incidents/Injuries: Jo-Ann has received five reports of chairs collapsing when the legs were not locked. No injuries have been reported.
Description: This recall involves the SmartSpace Papasan chair. The silver metal frame chair has a circular fabric seat that sits on a 10-inch-tall base. The fabric seat used with the papasan chairs was sold in purple, pacific, lime yellow and gray. A hangtag on the chair reads "Smartspace Papasan Chair - 1 piece."
Sold at: Jo-Ann Fabric and Craft stores nationwide from June 2008 through September 2008 for about $25.
Remedy: Consumers should immediately stop using the chair until a warning label, instructing consumers to engage the legs in the locked position before use, has been obtained from Jo-Ann Fabric and Craft Stores.
