Name of Product: Frameworx Table Height Chairs and Bar Stools

Units: About 22,400

Manufacturer: Brunswick Bowling & Billiards Corp., of Lake Forest, Ill.

Hazard: The down shaft on the seat plate assembly can fracture, which can cause an occupant of the seat to fall.

Incidents/Injuries: Brunswick has received reports of 20 incidents in which the seat plate assembly has failed. Three injuries resulting from falling from a seat have been reported.

Description: The recalled chairs and bar stools were sold under the model name Frameworx. The models affected are short table height chair model number 53-861025-000 and tall bar stool model number 57-860979-000. The model name and model numbers are not written on the chairs. The model number is only written on the original shipping carton, packing list or invoice. The chairs and bar stools have a plastic bucket-style seat bolted to a steel seat plate assembly that connects to the down tube of the seat.

Sold by: Distributors of Brunswick Bowling furniture selling to bowling center operators. This product was not sold to the general public. The chairs were sold from September 1995 through May 2004.

Remedy: Bowling centers owning these table height chairs and bar stools have been directly notified about this recall. Owners of the bar stools and table height chairs should stop using these products immediately and contact Brunswick to receive a free repair kit.

Name of Product: Homeroom Bedroom Collection

Hazard: The small cam lock covers can be easily removed, presenting a choking hazard to young children.

Incidents/Injuries: Hold Everything has received one report of a toddler removing a hardware cover from a dresser. No injuries have been reported.

Description: This recall involves plastic cam hardware covers used to cover the hardware on the Homeroom bedroom collection. Sold in a natural stain maple veneer, the collection includes a bed, dresser, nightstand, toy chest and shelving systems.

Sold at: HoldEverything.com and through the Hold Everything catalog nationwide from July 2004 through March 2006 for between $75 and $995.

Manufactured In: Denmark

Remedy: Consumers should immediately remove the hardware covers and dispose of them.

Name of Product: LYCKSELE Chair Bed and Sofa Bed

Manufacturer: IKEA Home Furnishings, of Plymouth Meeting, Pa.

Hazard: Fingers can become caught in the folding mechanism of the chair bed/sofa bed, posing a laceration and/or amputation hazard.

Incidents/Injuries: IKEA has received one report of a consumer who has been injured when attempting to operate the folding mechanism. The incident involved a fingertip amputation.

Description and Models: This recall involves LYCKSELE chair beds and sofa beds produced before July 2005 and date stamped (year/week) 0526 and earlier. The frames are constructed of iron and have natural wood slats. IKEA, the article number (700-326-82 chair bed and 900-326-81 sofa bed), and a four digit date stamp are printed on a white sticker located on the underside of the product.

Sold at: IKEA stores nationwide and on the Ikea Web site from April 1999 through June 2006 for $40 (chair bed) and $90 (sofa bed).

Manufactured In: France, Bulgaria, China and Romania

Remedy: Consumers who have a LYCKSELE chair bed or sofa bed date stamped 0526 or earlier and which is missing safety covers for the folding mechanism should call or visit their local IKEA store to receive free safety covers, which fit over the folding mechanism to prevent finger access. LYCKSELE chair beds and sofa beds produced since July 2005 (date stamp 0527 and later) were sold with safety covers.

Name of Product: Mainstays Associate Office Chairs

Hazard: The legs and backs of these chairs can break, and the chairs can easily tip over, posing a fall hazard to consumers.

Incidents/Injuries: Wal-Mart has received nine reports of chairs breaking and two reports of tipping. There have been seven reported injuries, including a broken wrist.

Description: The recall involves the Mainstays Associate office chair with a five-star nylon base with dual wheel casters and pneumatic seat height adjustment. The chair has a fabric cushioned seat and back that is black/grey in color. The armrests are shaped in half-circle loops. Under the seat is a UPC Code Label #9501401610 with printed Model #VCR1022.

Sold at: Wal-Mart stores exclusively nationwide from April 2003 through April 2006 for about $36.

Manufactured In: China

Remedy: Consumers should immediately stop using the product and return it to Wal-Mart for a full refund

Name of Product: Dragonfly, Petunia, Iris and Rose Torchiere Lamps

Units: About 2,200

Hazard: An electrical problem can cause the torchiere lamps to spark, smoke or burn, posing a fire or burn hazard to consumers.

Incidents/Injuries: ValueVision has received five reports of the recalled lamps sparking, smoking and/or burning, including minor property damage. One minor burn injury has been reported.

Description: The torchieres are Tiffany-style lamps with glass shades with either dragonfly, petunia, iris or rose designs.

 

• The Dragonfly torchiere has a dragonfly design glass shade. It was sold as part of a three-lamp set including a floor lamp, a table lamp and a torchiere lamp. Only the torchiere lamp in the set is included in this recall. The Dragonfly torchiere was sold as item numbers V38109 and K18266. The item numbers are located on the products' packaging.
• The Petunia torchiere has a pastel floral design glass shade and was sold individually as item number V38762.
• The Iris torchiere has an iris design glass shade and was sold individually as item number V38763.
• The Rose torchiere has a rose design glass shade and was sold individually as item number V38764.

 

Sold at: ShopNBC, which is ValueVision's television home-shopping channel, and at www.shopnbc.com between November 2005 and April 2006 for about $200 for the set that includes the Dragonfly lamp, and about $50 to $90 for the other individual lamps.

Remedy: Consumers should immediately stop using the recalled torchiere lamps and contact ValueVision to receive a full refund. ValueVision is contacting consumers who bought these lamps directly.

Name of Product: Herrington Swivel Rocker

Hazard: The recalled swivel rockers can be assembled incorrectly, causing them to break at the base or tip over backward easily. This poses a fall hazard to consumers.

Incidents/Injuries: LG Sourcing received nine reports of injuries including bruises and abrasions when the rocker broke or tipped over. One individual reported having a slight concussion. All of the reported injuries involved display models at Lowe's stores.

Description: The rocker is part of the Herrington collection of patio furniture. The rocker is dark brown with a brown cloth seat, has a swivel base and a cut out pattern on the metal backrest. The item number, 119178, is printed on the rocker's packaging and on the assembly instructions. The rocker came in two sections, a base and a seat, and customers were required to bolt the seat to the base.

Sold exclusively at: Lowe's stores nationwide from October 2005 through February 2006 for about $420 for a set of two rockers.

Manufactured In: China

Remedy: Consumers should stop using the recalled rockers immediately and return them to any Lowe's store for a full refund.

Name of Product: Computer Desk and Chair Set

Hazard: The seat on the chair can break and fall through during use causing an individual to fall and suffer injuries.

Incidents/Injuries: New Focus Marketing has received reports of two injuries involving chairs breaking. Injuries include a consumer who received bruises to the arm and shoulder, and another consumer who suffered a minor back injury.

Description: The recalled three-piece set includes a desk, computer stand and chair. Model number NF913232 is located on the product's box. The chair in the office set is gray with a black seat and a silver tubular metal frame.

Sold at: Office Depot stores nationwide from June 2005 through August 2005 for about $80 for the set.

Manufactured in: China

Remedy: Consumers should stop using the chair immediately and contact Office Depot to receive a gift card for the full price of the desk set upon receipt of the chair.

Name of Product: Mainstays Love Seat Rocker and Porch Rocker

Hazard: Poor construction and over-curvature of the chair's runners can cause instability, imbalance, fracturing of the wood and tip-over during use. This poses a fall hazard to consumers.

Incidents/Injuries: Wal-Mart has received 55 incident reports including 45 injuries. Those injuries include a cut in the leg requiring 16 stitches, a slight concussion, fractured ribs, wrist and cervical/lumbar sprains, upper back injuries, a pinched nerve, a shoulder joint tear and one incident in which a pregnant woman began having contractions after the display chair in which she was sitting flipped over backwards. Many of the injuries occurred on display models in Wal-Mart stores.

Description: The recall involves three models of rocking chairs, the Mainstays Love Seat Rocker (Model IT-13380) and the Mainstays Porch Rocker (Model IT-13379 and IT-13270). The Love Seat Rocker is wooden, white and seats two persons. The back support frame and the seat are made of slats. "Made in China" is printed on a sticker under the seat. The wooden Porch Rocker is sold in white and "natural" (light brown) color and seats only one person. "Made in Indonesia" is printed on a sticker under the Porch Rocker's seat. They sit on curved runners. The model number is printed on the rocker's packaging.

Sold exclusively at: Wal-Mart stores nationwide and on Wal-Mart's Web site. The Love Seat Rockers were sold from May 2004 through September 2005 for about $100. The Porch Rockers were sold from April 2004 through March 2006 for about $50.

Manufactured in: China and Indonesia

Remedy: Consumers should immediately stop using these rocking chairs and return them to Wal-Mart for a full refund.

After the death of a 19-month-old baby in Myrtle Creek, Ore., the U.S. Consumer Product Safety Commission (CPSC) and Simplicity Inc., of Reading Pa., are renewing the search for recalled Aspen 3 in 1 Cribs with Graco logos. The child died on January 6, 2006, after two of the mattress support slats came out of his recalled crib. He became entrapped between the mattress and the footboard of the crib and suffocated.

CPSC and Simplicity Inc. announced the recall of about 104,000 Aspen 3 in 1 Cribs on December 21, 2005. The recall was conducted because the screws on the wooden mattress supports can come loose, allowing a portion of the mattress to fall. This poses a suffocation hazard to young children who can slide down and become entrapped between the unsupported mattress and end of the crib.

Prior to the report of this death, Simplicity Inc. received 14 reports of the mattress supports coming loose, including eight reports of entrapment. Five injuries were reported including scratches and bruises to the face and head, a strained neck and a report of a child turning blue.

Although the Graco logo appears on these products, the cribs were manufactured by Simplicity Inc. Consumers should only contact Simplicity about this recall.

The recalled cribs are made of wood and have wooden mattress supports. Only cribs with wooden mattress supports and with model number 8740KCW SC and serial number 2803 SC (made the 28th week of 2003) to 1605 SC (made the 16th week of 2005) are included in this recall. The model and serial number are printed on the envelope attached to the mattress support.

The recalled cribs were sold in department and children's product stores from August 2003 through May 2005 for about $130.
