Recent Death Prompts Renewed Search for Simplicity Cribs With Graco Logo
simplicity cribs gracoAfter the death of a 19-month-old baby in Myrtle Creek, Ore., the U.S. Consumer Product Safety Commission (CPSC) and Simplicity Inc., of Reading, Pa., are renewing the search for recalled Aspen 3 in 1 Cribs with Graco logos. The child died on January 6, 2006, after two of the mattress support slats came out of his recalled crib. He became entrapped between the mattress and the footboard of the crib and suffocated.

CPSC and Simplicity Inc. announced the recall of about 104,000 Aspen 3 in 1 Cribs on December 21, 2005. The recall was conducted because the screws on the wooden mattress supports can come loose, allowing a portion of the mattress to fall. This poses a suffocation hazard to young children who can slide down and become entrapped between the unsupported mattress and end of the crib.

Prior to the report of this death, Simplicity Inc. received 14 reports of the mattress supports coming loose, including eight reports of entrapment. Five injuries were reported including scratches and bruises to the face and head, a strained neck and a report of a child turning blue.

Although the Graco logo appears on these products, the cribs were manufactured by Simplicity Inc. Consumers should only contact Simplicity about this recall.

The recalled cribs are made of wood and have wooden mattress supports. Only cribs with wooden mattress supports and with model number 8740KCW SC and serial number 2803 SC (made the 28th week of 2003) to 1605 SC (made the 16th week of 2005) are included in this recall. The model and serial number are printed on the envelope attached to the mattress support.

The recalled cribs were sold in department and children's product stores from August 2003 through May 2005 for about $130.

Manufacturer: Stork Craft Manufacturing Inc., of Richmond, British Columbia, Canada

Hazard: The toy box's lid support can fail, allowing the lid to collapse suddenly. This poses a strangulation hazard to young children, and possible impact injuries to a child's head, neck, fingers or hands.

Incidents/Injuries: CPSC and Stork Craft have received two reports of the lid collapsing. No injuries have been reported. Stork Craft has received 11 consumer requests for replacement lid supports.

Description: The recall includes toy boxes with manufacture date codes between November 2002 and July 2004 and the following model numbers and colors: 01520-331 – white, 01520-334 – cherry, 01520-33C – cognac, 01520-33L – oak and 01520-33N – natural. The manufacture date codes, model numbers and colors are located on a label underneath the toy boxes. The toy boxes measure 27 inches long by 18 inches wide and 17 inches high. The boxes are constructed of pine.

Sold at: Wal-Mart and J.C. Penney Web sites, and at furniture, baby and juvenile stores nationwide from December 2002 through April 2005 for about $90.

Remedy: Consumers should immediately stop young children from using these toy boxes and contact Stork Craft for information to determine if their toy box lid support needs to be replaced. If so, the firm will provide a free replacement lid support.

CPSC, Pottery Barn Kids Announce Recall of Toy Chests

Manufacturer: Pottery Barn Kids of San Francisco, Calif.

Hazard: A problem with the lid support on the toy chest could cause the lid not to stay open, posing the risk of an impact injury to a child's head, fingers or hands.

Incidents/Injuries: No injuries or incidents have been reported.

Description: This recall includes the Cameron toy chest in model numbers 4825378 (Honey), 6252852 (Espresso) and 4825386 (White). The product name and model number is located on a label affixed to the back of the toy chest. Inside the lid of the toy chest is a blackboard and an extra storage shelf. The toy chests are made of American hardwood frame with veneer panels and measure 40x18x20 inches high.

Sold at: Pottery Barn Kids' Web site, catalog and retail stores nationwide from December 2003 through April 2005 for $349.

Remedy: Consumers should immediately stop young children from using these toy chests and contact Pottery Barn Kids to determine if their toy chest lid support needs replacement. Pottery Barn Kids will provide consumers with a free lid support replacement kit.

CPSC, Graco Children's Products Announce Recall of Toddler Beds

Hazard: A child's arm, leg or foot can become entrapped between the slats in the guardrails or footboard. This can result in broken bones, sprains and other injuries to young children.

Recall furnitureIncidents/Injuries: Graco has received reports of 77 entrapments. This resulted in 13 broken arms and legs, one broken foot, a sprained ankle and 54 other injuries including bruised, scratched and swollen limbs.

Description: The recalled Graco toddler beds are white plastic and steel with openings between the slats in the guard rails and footboard. The beds were sold under the names "Cozy Toddler Bed," "Glow-in-the-Dark Toddler Bed" and "Classic Toddler Bed." There is a label on the guardrail or leg containing one Recall furnitureof the following model numbers: 8801, 8801WR, 8821, 8824, 8828, 8833, 30066, 34434 or 11030, a serial number and the manufacturer's address. Model 8828's headboard has a blue sky with a yellow moon and stars. Model 8801WR has red legs. "Graco" is printed on all of the beds' footboards.

Sold at: Discount, department and juvenile product stores nationwide from February 1994 through March 2001 for between $50 and $70.

Recall furnitureRemedy: Consumers should remove the guardrails from the recalled Graco toddler beds immediately and call the firm to receive a free retrofit kit. The kit includes custom-designed mesh coverings that must be attached to the guardrails and footboard to prevent entrapment. The mesh coverings will be available in six to eight weeks.

Manufacturer: Commend Co. Ltd., of Fair Lawn, N.J.

Hazard: The wooden chairs can break, posing a fall hazard to consumers.

Incidents/Injuries: Commend has received one report of a chair breaking. In this report, there was a fall and minor injury.

Description: The chairs and table are sold unassembled as part of a boxed five-piece dining set. The words "HD Designs 5 Piece Dining Set" and item number BFR3917HO are written on the box. The chairs and dining table are honey oak wood and the seats have beige vinyl padding.

Sold at: Fred Meyer stores in Idaho, Washington, Oregon and Alaska; Fry's stores in Arizona; and Smith's Marketplace stores in Utah from April 2004 to October 2004 for about $200.

Manufactured in: Thailand

Remedy: Consumers should immediately stop using the product and contact Fred Meyer to receive a store credit.

CPSC, Poundex Associates Corp. Announce Recall of Dining Chairs

Manufacturer: Poundex Associates Corp., of City of Industry, Calif.

Hazard: The welds on the chair's rear legs can crack and break apart, posing a fall hazard to consumers.

Incidents/Injuries: Poundex Associates Corp. has received one report of a chair breaking, involving one report of a minor injury.

Description: The recalled dining chair has a dark bronze metallic frame, birch colored wooden back and gold colored upholstered seat. The front and rear legs have a curved shape. The label under the seat reads in part "Imported by Poundex Associates Corp." The recalled dining chair is approximately 42 inches high and 17 inches wide.

Sold at: Independent furniture stores nationwide from January 2004 through September 2004 for about $50 each. The recalled chairs were mainly sold as part of a five-piece dining set with a table for about $400 per set.

Manufactured in: China

Remedy: Consumers should stop using the chairs immediately and contact Poundex to receive a free rear leg replacement kit.

CPSC, IKEA Home Furnishings Announce Recall of Fargglad Multi-Color Children's Chair

Hazard: Choking hazard from red plastic feet that can detach from chair legs.

Incidents/Injuries: None reported.

Description: Fargglad multi-color children's chairs have the article number 400.548.40 and the supplier number (either 16592 or 17128) on a label on one of the chair's rear legs, or on the underside of the chair seat. The chair has a tubular steel frame with woven multi-color solid plastic piping used for the chair's seat and back. The plastic feet are red.

Sold at: IKEA stores sold the recalled children's chair from May 2003 through September 18, 2004, for about $15.

Manufactured in: Vietnam (supplier number 17128) or China (supplier number 16592)

Remedy: Full refund. Take the Fargglad multi-color children's chair away from young children immediately and return it to the "Returns and Exchanges" department at any IKEA U.S. store for a full refund.

CPSC, Sauder Woodworking Co. Announce Recall of TV/VCR Carts

Manufacturer: Sauder Woodworking Co., of Archbold, Ohio

Hazard: The carts can tip over and the television can fall off, posing a risk of serious injury or death if the TV and cart fall on a child.

Incidents/Injuries: Sauder Woodworking received one report of a cart tipping over, though no injuries have been reported.

Description: The recalled TV/VCR carts are white, light brown and light reddish brown; have decorative hardware and trim; and are about 29 inches wide, 17 inches deep and 25 inches high. The carts are equipped with a top shelf for a television 20 inches or smaller and a middle shelf for a VCR and a lower storage area. The recalled carts include models 3355, 6355, 7755, 9855 and 9755, which can be found on the instruction book that came with the cart.

Sold at: Department, discount and home electronic stores nationwide sold the 3355, 6355 and 7755 models from October 1991 through May 1999 for between $80 and $100. The 9855 and 9755 models were sold exclusively at Target stores.

Manufactured in: United States

Remedy: Consumers should stop using the TV/VCR carts immediately and do the following:

1. Remove the television and all contents.

2. Turn the cart over and remove the four casters from the bottom of the cart. This will bring the tip-stability of the cart into compliance with the latest industry standards.

3. Contact Sauder Woodworking Co. to receive a free retrofit kit and safe use information.

When updated, this cart will be suitable for use with TVs weighing 50 pounds or less. This includes most TVs that are 20 inches or smaller.

CPSC, IKEA Home Furnishings Announce Recall of Glass-Door Wall Cabinets

Manufacturer: IKEA Home Furnishings, of Plymouth Meeting, Pa.

Hazard: Some of these glass-door wall cabinets have the wrong sized screws for the safety bracket, which can result in the cabinet not being properly secured to the wall. The cabinets can fall and injure nearby consumers.

Incidents/Injuries: IKEA has received two reports of cabinets falling. No injuries have been reported.

Description: The glass-door wall cabinets have "BERTBY," the product article number (380.472.10) and supplier number (14709) written on a label on the underside of the bottom panel or on the top side of the top panel. The cabinets are about 67 inches high.

Sold at: IKEA stores nationwide from January 2002 through September 9, 2004, for about $100.

Manufactured in: Italy

Remedy: IKEA is offering a free repair kit. Consumers should contact IKEA to determine if their wall cabinet needs this repair.
