Manufacturer: Nike Inc., of Beaverton, Ore. They were manufactured in China.
Hazard: The chin strap's plastic cup can break as a result of contact, exposing
the player to facial or head injuries.
Incidents/Injuries: Nike has received 18 reports of the chin strap breaking, including two reports of concussions, two reports of facial lacerations requiring stitches and a report of a broken nose.
Description: The recalled football helmet chin strap consists of a plastic cup with a foam liner, straps and four metal snaps. They were sold in both youth and adult sizes and come in black/gray and white/gray color combinations. The Nike Swoosh trademark is printed on the outside of the chin cup and DRI-FIT™ is printed on the foam liner. "Made in China" and one of the following four style numbers is printed on the inside of the strap: FA0016 046, FA0016 130, FA0021 046 or FA0021 130.
Sold at: Sporting good stores nationwide and on the firm's Web site www.Nike.com from April 2006 through October 2007 for about $20.
Remedy: Consumers should stop using the chin strap immediately and contact Nike for a product voucher redeemable at www.nikestore.com.

Name of Product: 2006 Line X-Fly and Line Pro Ski Boards
Manufacturer: Line Skis, of Seattle, Wash.
Hazard: Screws installed improperly can cause the bindings to come loose or pull
off the ski board during use, causing the skier to lose control or fall and suffer
injuries.
Incidents/Injuries: The firm has received one report of the screws that hold the binding to the ski board pulling out during use. No injuries have been reported.
Description: The 2006 Line X-Fly and Line Pro model ski boards were sold in 90 and 99 centimeter lengths. The Pro model is a twin-tip ski that is turned up at both ends. "Pro 90" or "Pro 98" is printed on the tail. The X-Fly is a unidirectional ski that is turned up on one end. "Fly 90" or "Fly 99" is printed on the tail.
Sold at: Ski, snowboard and sporting goods stores from September 2006 through January 2007 for about $180. They were manufactured in Taiwan.
Remedy: Consumers should immediately stop using the recalled ski boards and return them to the place of purchase for a full refund. If consumers are unable to return the ski boards to the place of purchase, contact the firm for instructions.

Name of Product: Cybex or Trotter Treadmills
Manufacturer: Cybex International, Inc., of Medway, Mass.
Hazard: During repairs associated with the October 8, 2003, recall, wire nuts
were installed improperly, causing the treadmills to overheat and posing a fire
hazard to consumers.
Incidents/Injuries: Cybex International has received five reports of treadmills overheating or catching fire, resulting in damage to the treadmill. No injuries have been reported.
Description: This recall involves the treadmills "Cybex 400T," "Cybex 410T," "Trotter 510," "Trotter 525," "Trotter 535" and "CXT+" that were recalled (on October 8, 2003) and later repaired. The treadmills are black with gray coloring, have rectangular uprights and measure 69 inches long and 30 inches wide. The brands and models are written on the display panel. The "CXT+" model does not bear the brand Cybex or Trotter.
Sold at: Cybex International and Cybex dealers nationwide from September 1993 through October 2001 for between $3,300 and $4,000. They were manufactured in the United States.
Remedy: Consumers whose treadmills were included in the 2003 recall and were repaired should immediately unplug and stop using the treadmills. Consumers with recalled units will be notified directly by Cybex. Consumers should call Cybex directly to schedule a free repair.

Name of Product: Reynolds UL Bicycle Forks
Manufacturer: MacLean Quality Composites, d.b.a. Reynolds, of West Jordan, Utah
Distributors: Quality Bicycle Products, of Bloomington, Minn., and Security Bicycle Accessories, of Hempstead, N.Y.
Hazard: The fork tips could separate from the fork legs, causing the wheel to
come loose from the fork while riding. This could pose a serious fall hazard to
riders. No injuries or incidents have been reported.
Description: This recall involves the Reynolds UL bicycle fork models with 43 mm and 50 mm rakes and with serial numbers 09100 through 10403. The serial number decal is located on the steer tube. To see the serial number, the fork must be removed from the frame. Some recalled forks are black, have "Reynolds" printed on them and have a gray and white stripe painted on the legs of the fork. Other recalled forks were painted with a different paint scheme and do not have "Reynolds" printed on them. To determine if a fork is included in this recall, consumers should contact the company.
Sold at: Independent bicycle retailers nationwide from October 2006 through May 2007 for about $500. The recalled forks were included in custom 2007 bicycle frame kits from the following manufacturers: Independent Fabrication, Litespeed, Lynskey Performance Designs LLC., Merlin, Parlee, Rock Lobster, Spectrum Cycles, Strong Frames and Vicious Cycles. They were manufactured in United States.
Remedy: Consumers should stop using the bicycles equipped with the recalled forks immediately and contact Reynolds for a free replacement fork.

Name of Product: Kong Maxx Ladder Stands
Manufacturer: Gorilla Inc., of Flushing, Mich.
Hazard: The ladder stands can become unstable while in use, causing the user
to fall. The addition of one ladder section to the stand makes the platform farther
away from the bracing. When the stand is at its full height, the distance between
the bracing and the platform may result in platform instability prior to attachment
to and/or detachment from the tree.
Incidents/Injuries: Gorilla Inc. has received two reports of consumers who fell from these ladder stands and suffered injuries to the back and knee.
Description: This recall involves Gorilla Kong Maxx Ladder Stands with model number 43032 and batch numbers G051507, G051507D, G061507D, G071507D and G081507D. Model numbers are printed on the box only. Batch numbers are stamped on the vertical rail of the first ladder section. The stand is 19 feet to the shooting rail, or 16 feet, 8 inches to the platform. The foot platform measures 17 1/2 inches wide. This is the only Gorilla, Inc. ladder stand with these dimensions.
Sold at: Gander Mountain stores nationwide from July 2007 through September 2007 for about $180. They were manufactured in China.
Remedy: Consumers should immediately stop using the ladder stands and contact Gorilla Inc. to receive a refund.

Name of Product: Rocky Mountain-Solo Bicycles

Hazard: The head tube can detach from the rest of the frame, posing a fall hazard
to consumers.
Incidents/Injuries: Procycle Group Inc. has received two reports of head tubes detaching from the frames, resulting in shoulder and facial injuries.
Description: This recall involves the Rocky Mountain Bicycles brand of Solo bicycles. The models included in this recall are 2006 SOLO CR50, 2006 SOLO CR70 and 2007 SOLO CR LTD. The bicycles have a full carbon Columbus frame. "Rocky Mountain" and "Solo" are printed on the head tube.
Sold by: Independent bicycle dealers nationwide from August 2006 through July 2007 for between $2,000 and $5,850. They were manufactured in Taiwan.
Remedy: Consumers should stop using the bicycle immediately and return the frame to the bicycle store where purchased to receive a free replacement frame, a full refund or credit.

Name of Product: Oro Disc Brakes used on bicycles

Hazard: The brake's hand lever can separate, resulting in loss of braking. This can cause the rider to lose control of the bicycle, posing a risk of injury to riders.
Incidents/Injuries: Formula Brake USA has received six reports of hand levers detaching. No injuries have been reported.
Description: This recall involves Oro disk brakes typically installed on mountain bikes. The brake is operated by a hand lever, which is mounted to the handlebar. Included in the recall are the Oro K18, Oro K24, Oro Puro and Oro Bianco models with serial numbers less than 295237. The serial number can be located on the brake's body above the "Made in Italy" label.
Sold at: Bicycle specialty stores nationwide from May 2005 through July 2007 for between $180 and $320. They were manufactured in Italy.
Remedy: Consumers should stop using bicycles equipped with the recalled brakes and contact any Formula Brake dealer to receive a free repair.

Name of Product: 2007 Huffy "Howler" and "Highland" Bicycles

Hazard: The bicycle crank can unexpectedly come off, causing the rider to lose
control, fall and suffer serious injuries.
Incidents/Injuries: Huffy has received two reports of the crank coming off, resulting in one injury.
Description: The recall involves 2007 model year, multi-speed bicycles with 26-inch, 24-inch or 20-inch wheels. The "Howler" was sold in black (model K3587, boy's model), blue (model K4587, men's model) and red (model K6587, men's model). The "Highland" was sold in white (model K4597, women's model) and blue (model K6597, women's model). The name "Howler" or "Highland" is printed on the frame of the bicycle, and the name "Huffy" is on the front of the frame. Model numbers are located on a label on the bottom of the frame where the crank is attached to the bicycle.
Sold at: Kmart stores nationwide from May 2007 through July 2007 for between $80 and $100. Bicycles were manufactured in China.
Remedy: Consumers should stop using the recalled bicycles immediately and contact Huffy Corp. to receive instructions on tightening the crank.

Name of Product: Double Hammocks

Hazard: The wooden rods at the head and foot of the hammock used to extend the
fabric can break, causing the hammock to collapse and the user to fall to the
ground.
Incidents: Collections Etc. is aware of five reports of the wooden rods breaking, resulting in four reports of injuries including minor cuts and sore necks and backs.
Description: This recall involves hammocks made with a solid green fabric and white support ropes. The hammocks' green fabric measures 73 inches long by 46 inches wide and supports a maximum of up to 350 pounds.
Sold through: Web sites and catalogs nationwide from May 2007 through August 2007 for about $20. They were manufactured in Taiwan.
Remedy: Consumers should stop using the hammocks immediately and contact Collections Etc. for a full refund.

Name of Product: Cub Scouts Totem Badges

Hazard: The surface paints on the badges contain excessive levels of lead, violating
the federal lead paint standard. No injuries or incidents have been reported.
Description: The recalled Cub Scout Totem Badges measure 4 1/2 inches x 3 inches with a square portion measuring 2 1/4 inches x 2 1/4 inches. The Boy Scout emblem is printed on the badge along with a wolf and bear. "PROGRESS TOWARD RANKS" is also printed on the badge.
Sold at: Boy Scouts of America retail outlets nationwide from January 2000 through September 2007. They were manufactured in China.
Remedy: Parents should immediately remove the badges from clothing and keep the badges away from children. Contact the firm for instructions on obtaining a free replacement badge.
